Envolve Pharmacy Solutions is simplifying the prescriber process with streamlined Prior Authorization (PA) that can be accessed online via any computer through: CoverMyMeds. CoverMyMeds automates drug prior authorizations for any medication and allows prescribers to begin the PA process electronically.

To begin, simply locate the correct form, complete it online, and then submit the PA form to Envolve Pharmacy Solutions via fax.

To begin a PA request electronically:

1. Select your state
2. Enter the drug you are submitting for PA
3. Select the member’s health plan
4. Click the “Start Request” button

Fax completed form to the number listed on the PA form.
