Please contact the Envolve Pharmacy Solutions Customer Service Center at (800) 460-8988 for next steps on how to approach processing the member’s claim.  We can also provide an opportunity for the member to identify themselves at that time and order a replacement card to be later mailed to them.

You will need to download, print and fill out the Contract Request Form.  Once you have completed and faxed the form to the number printed on the document, your request will be processed in a timely fashion and you will receive follow-up communication from our Pharmacy Network Operations Department.
