Prior Authorization

CoverMyMeds is Envolve Pharmacy Solutions’ preferred way to receive prior authorization requests. Visit to begin using this free service.

There are three options for submitting a Prior Authorization (PA) form:

  1. Submit PA electronically through CoverMyMeds
  2. Complete and submit our online form
  3. Locate the appropriate form from our plan websites. Once you have printed the form and completed all appropriate fields, please fax the completed form to the number listed on the form.

PA Review:

Depending on the type of medication and whether any additional information is needed to process the PA request, the review process can take one day, a few days, or longer. Your assigned Envolve Pharmacy Services PA contact will guide you through the process if additional information is required. Requesting an Appeal:

If a PA request is denied, instructions for the Appeals process will be included within the communication sent informing you of the decision.
